Our Process

How we do what we do

We share our knowledge and expertise with you to help you choose the right options for your aged care needs.

1

We get to know you

We visit you in your home and listen while you tell us about yourself. We find out about your life, what is important to you and your future goals.

2

Apply for care

Once you have obtained your ACAT assessment it is time to find a suitable care home. We will explain the types of care available. You will tell us what area you would you like to live in, your funds available and when you want to move. We will research homes in your desired area and provide you with a report with our findings.

3

Make the transition

Making the transition into aged care can be extremely confronting. You can often experience a feeling of grief and loss, negative perceptions of aged care facilities and loss of independence. There are some strategies to help address these issues including: Acceptance of changed circumstances; motivation to engage; and increased social interaction compared to home. One of the other problems with the transition is that you can become depressed. It is very important that you discuss your concerns with your doctor.

4

Continual Support

We can continue to visit you in your chosen facility for three months to ensure you have settled in well and to address any issues you have with the facility.
